Upon the death of the Retiree and/or Surviving spouse, what do family members need to do?
In the event of death of the retiree and/or surviving spouse, the family members should do the following:
Notify the Retirement Plan for the CTA Employees at 312.441.9694 of the retiree's death and file start the claim process
Submit an original Death Certificate to the Plan

As of Monday, June 22nd, our office is open on an appointment-only basis. We are complying with City of Chicago capacity restrictions, so we cannot accept walk-ins at this time
Please do not come to the office without an appointment.
If you have a question about benefits from the Retirement Plan, please call us at 312.441.9694 and leave your name, phone number, and badge # (if applicable). We will return your call within one business day. We can also set up in-person or videoconference appointments with you as needed.
If you have a scheduled appointment with a staff member, you will be required to wear a mask, complete a COVID-19 symptom checklist, and have your temperature taken before you can enter the office.
